3. Complete the registration and entry form with your full name, phone number, and state/territory of residence.
What are the prizes?
The prize is a $1,000 Eftpos gift card. There will be one winner selected randomly.

Notification of Winners
The winner will be notified by phone and email on Sunday 7 July 2024. The winner's name and location will be published on the Koko Black website.
Prize Claim Date
The prize must be claimed by 7 October 2024.
Unclaimed Prize Arrangements
If the prize is not claimed by the prize claim date, a further draw will take place on 8 October 2024 at 10:00 am AEST at Koko Black, Level 1, 110 Gaffney Street, Coburg VIC 3058. The new winner will be notified by email or telephone on the day of the unclaimed prize draw.
Additional Prize Terms
The gift card is valid for 3 years from the date of issue and is subject to the terms and conditions of the provider.
